Referring a Patient

Hummingbird Hospice accepts referrals from healthcare professionals or loved ones at any stage of a patient’s illness.  We encourage referrals in the early stages as evidence suggests the earlier a referral is made to specialist hospice care services the better the quality of life for the patient. Early referrals enable us to make contact with the patient and their family, to develop a relationship and understanding of their needs, and to help with the coordination of their care.

It can be difficult to determine when an individual is medically appropriate for hospice care. Several signs and symptoms indicate that a patient is medically appropriate for care. The following guidelines can help determine when to make a referral:

  • Uncontrolled or chronic symptoms that interfere with quality of life

  • Progressive decline in functional capability

  • Unintentional, progressive weight loss greater than 10 percent in the past six months

  • May have multiple chronic comorbidities

  • Dependence on assistance for most cares

  • Recurrent infections

  • Recurrent ED visits or hospitalizations

  • Non-healing wounds

 

We can help you determine if your patient qualifies for hospice care, or broach the topic with your patients. 

On referral, the patient will be assessed by one of our Registered Nurse Specialists to understand their needs and to ensure the most appropriate care can be arranged.  All referred patients also receive access to our 24-hour advice line.

Once a referral is received, it is Hummingbird's goal to obtain all the required information and schedule a visit within 24 hours or less. No one should be left in fear or without care.
